In most crisis situation, often the people in charge refused to accept the need to change. It may be due to refusal to admit that a mistake may have been made or perception that mistakes have been made.

Then the situation worsen and the organisation is reacting to wavws of bad publicity.

The reputation is then destroyed even at the end the organisation eventually may do what they should have done in the first place.

The best example would be the change of PM in Malaysia.
